    Dear Blum,

    Can you check the Qlogic settup (Setup->Fibre Channel)

    Also, Be aware that MPIO mode for the QLOGIC HBA FC does not work in the DEFAULT kernel. Please switch to the BACKUP kernel to use this mode.
